London- Insulin resistance is associated with stroke, according to a study of more than 100,000 people with Type 2 diabetes (T2D), presented at the recent annual meeting of the European Association for the Study of Diabetes (EASD).
Insulin resistance is when the body's cells don't respond properly to insulin and can't easily take up glucose from blood a key feature of T2D and levels vary from patient to patient.
The higher the insulin resistance, the greater the risk of stroke, revealed the study led by a joint team at the Karolinska Institute, Gothenburg University and the National Diabetes Registry in Sweden.
The team used the estimated glucose disposal rate (eGDR) as a measure of insulin resistance. eGDR has previously been shown to be a good proxy for insulin resistance and is calculated using a formula that factors in a patient's waist circumference, HbA1c (average blood sugar level) and whether they have high blood pressure.
Health records were used to calculate the eGDR of 104,697 T2D patients in Sweden. They were followed up for an average of 5.6 years, during which 4,201 (4 per cent) had a stroke.
Those with the lowest insulin resistance (the highest eGDR) were 40 per cent less likely to have a stroke than those with the highest insulin resistance.
The study also found that higher insulin resistance was linked to a higher risk of death after a stroke. Those with the lowest resistance were 28 per less likely to die during the follow-up period than those with the most severe insulin resistance.
Further analysis showed high blood pressure to be more strongly linked to stroke than waist circumference or HbA1c.
"We found that in individuals with type 2 diabetes, a low eGDR, a simple measure of insulin resistance, was associated with an increased risk of stroke and mortality," said Alexander Zabala from the Karolinska Institute.
"eGDR could be used to help T2D patients better understand and manage their risk of stroke and death," Zabala added. (agency)

New York - Women who contract Covid-19 during pregnancy are at significantly higher risk of developing pre-eclampsia, the leading cause of maternal and infant death worldwide, according to a new study.
Pre-eclampsia is a sudden increase in blood pressure after the 20th week of pregnancy.
The study, published in the American Journal of Obstetrics and Gynecology, showed that women with SARS-CoV-2 infection during pregnancy had 62 per cent higher odds of developing preeclampsia than those without the infection during pregnancy.
"This association was remarkably consistent across all predefined subgroups. Moreover, SARS-CoV-2 infection during pregnancy was associated with a significant increase in the odds of pre-eclampsia with severe features, eclampsia and HELLP syndrome," said Roberto Romero, Professor of Molecular Obstetrics and Genetics at the Wayne State University School of Medicine.
HELLP syndrome is a form of severe pre-eclampsia that includes hemolysis (the rupturing of red blood cells), elevated liver enzymes and a low platelet count.
The team published their findings after reviewing 28 previous studies that included 790,954 pregnant women, including 15,524 diagnosed with Covid-19 infection.
"Both asymptomatic and symptomatic infection significantly increased the risk of pre-eclampsia," Romero said.
"Nevertheless, the odds of developing pre-eclampsia were higher among patients with symptomatic illness than among those with asymptomatic illness."
Pre-eclampsia warning signs, in addition to elevated blood pressure, can include headaches, swelling in the face and hands, blurred vision, chest pain and shortness of breath.
The condition is responsible for 76,000 maternal deaths and more than 500,000 infant deaths every year, according to estimates from the Pre-eclampsia Foundation.
The babies of pre-eclamptic mothers are affected by the condition and may develop intrauterine growth restriction or die in utero.
The researchers said health care professionals should be aware of the association and closely monitor pregnant women who are infected for early detection of pre-eclampsia.
A separate study, published in the American Journal of Obstetrics and Gynecology-Maternal-Fetal Medicine, showed that women who receive the mRNA Covid-19 vaccine during pregnancy pass high levels of antibodies to their babies.
The study of 36 newborns whose mothers received either the Pfizer-BioNTech or Moderna Covid-19 vaccine during pregnancy showed that 100 per cent of the infants had protective antibodies at birth.

According to a recent study published in international health journals around the world, constant exposure to traffic noise increases the risk of dementia among aged population.
Each year, September 21 is commemorated as the World Alzheimer's Day, and Alzheimer's disease has been found to be the commonest cause of dementia.
Doctors say with India fast racing towards becoming the most populated country in the world, and with improved healthcare delivery mechanism, aged population is on the rise in the country. This section of the society is at the risk of developing age-related complications like dementia, which is often considered a serious mental problem caused by brain disease or injury, that affects the ability to think, remember and behave normally.
Commenting on the problem, Sritheja Reddy, Consultant Neurologist, Gleneagles Global Hospital believes that music usually has a soothing effect on individuals of all ages, but loud and persistent noise can cause mental disturbance, and could even trigger experiences of ill being among those people who are suffering from dementia or Alzheimer's related problems. "Cities are usually bustling with great activity during the day times and in the nights, but this could increase exposure to excessive noise, that can lead to short term impairments in cognitive function, particularly with respect to the ability to focus and remember. And the most important aspect here is that chronic exposure to noise pollution may increase the risk for dementia," the doctor said.
Abhinay M. Huchche, Consultant Neurologist, SLG Hospitals says that musical sounds could have a positive impact on people suffering from dementia, caused due to Alzheimer's disease." Listening to or singing songs can provide emotional and behavioral benefits for people with Alzheimer's disease and other types of dementia. Musical memories are often preserved in Alzheimer's disease because key brain areas linked to musical memory are relatively undamaged by the disease," he added.
Changala Praveen, Consultant - Neurophysician, Aware Gleneagles Global Hospital pointed out that ageing patients require extra attention, and those impacted by dementia require proper evaluation and management of the disease through a multidisciplinary approach. "Alzheimer's disease is the most common cause for dementia, and there are multiple reasons for aged population developing this problem. Constant exposure to loud and unsavoury sounds like traffic noise is also a major cause for the older people to develop dementia. People who have aged family members in the house must ensure the elderly are protected from loud noises, and this is the best solution to arrest complexities," he said.
Having exposure to high levels of noise during the night is especially concerning, as sleep is a critical period for mental and cognitive restoration. Fragmented sleep resulting from noise disturbance is associated with increased oxidative stress causes alterations in the immune system and increased systemic inflammation.

Bhubaneswar, Aug 3 (IANS) After a gap of three months, the Odisha government has asked all its offices to function with full strength."All departments of the state government and sub-ordinate offices in the state shall function with full strength of employees until further orders," the general administration & public grievance department said in an order issued on Tuesday.As most of the employees are now fully vaccinated, the state government has asked all of them to mandatorily attend the office regularly.The employees who are unable to get vaccinated for medical or some other compelling reason, may seek exemption from attending office from the head of office, which would be considered on a case-to-case basis, it said.Employees who are neither vaccinated nor exempted would not be permitted to attend office and their absence will be treated as willful, warned the government.However, all the government offices will remain closed on the second and fourth Saturdays of the month. In view of the second wave of Covid-19, the offices remained closed on all Saturdays.All the offices have been asked to follow issued instructions and guidelines to prevent spread of Covid infection and detection of any Covid case.On Tuesday, the state has reported 1,129 fresh Covid cases and confirmed 69 deaths, taking the death toll to 6,102. The State now has 14,325 active cases.Meanwhile, temples have been allowed to reopen for devotees in some districts including Puri, Sambalpur and Cuttack. New Delhi, June 30 (IANS) The Union Ministry of Health and Family Welfare on Wednesday said India's COVID-19 vaccination coverage has increased to 33.28 crore.India's cumulative vaccination coverage had crossed 33 crore on Tuesday. A total of 33,28,54,527 vaccine doses have been administered through 44,33,853 sessions, as per the provisional report till 7 a.m. Wednesday while 36,51,983 vaccine doses were administered in the last 24 hours.India has reported 45,951 new cases in the last 24 hours. Less than 50,000 daily new cases have been reported since three continuous days. This is a result of sustained and collaborative efforts by the Centre and the states/UTs.India has also been witnessing a continuous fall in the active caseload. The country's active caseload now stands at 5,37,064.A net decline of 15,595 was witnessed in the last 24 hours and active cases are now only 1.77 per cent of the country's total positive cases.As more people recover from COVID-19, India's daily recoveries have continued to outnumber Daily New Cases for 48 consecutive days now.Out of the people infected since the beginning of the pandemic, 2,94,27,330 people have already recovered from COVID-19 and 60,729 patients have recovered in the last 24 hours. This constitutes an overall recovery rate of 96.92 per cent, which is showing a sustained increasing trend.With substantially ramped up testing capacity across the nation, a total of 19,60,757 tests were conducted in the last 24 hours in the country. Cumulatively, India has conducted over 41.01 crore (41,01,00,044) tests so far.The testing capacity has been enhanced across the country and a continued decline in weekly case positivity is noticed. Weekly positivity rate is currently at 2.69 per cent while the daily positivity rate stands at 2.34 per cent. It has remained less than 5 per cent for 23 consecutive days now.--IANSmiz/kr
