आज के समय में फिलहाल कैंसर का सबसे कारगर उपचार कीमोथेरेपी है। हालांकि कीमोथेरेपी शरीर में कैंसर के सेल पर हमला करने के साथ-साथ अन्य सामान्य कोशिकाओं पर भी हमला करता है जिससे रोगी की हालत कुछ दिनों के लिए बहुत खराब हो जाती है। भारतीय शोधकर्ताओं ने अब एक ऐसी तकनीक खोज निकाली है जिसकी थेरेपी के दौरान केवल कैंसर के सेल पर हमला किया जाएगा और शरीर की बाकी कोशिकाओं को नुकसान नहीं पहुंचेगा।
केंद्रीय विश्वविद्यालय बीएचयू के शोधकर्ताओं द्वारा विकसित माइक्रो आरएनए विशेष रूप से सर्वाइकल कैंसर कोशिकाओं को मारता है। वर्तमान में सर्वाइकल कैंसर के लिए उपलब्ध इलाज कीमोथेरेपी और रेडियोथेरेपी हैं, लेकिन इसका गैर कैंसर कोशिकाओं पर भी प्रभाव पड़ता है, जो काफी हानिकारक और विषाक्त है। अध्ययन के निष्कर्षों से भविष्य में सर्वाइकल कैंसर के इलाज के लिए माइक्रो आरएनए की ऐसी थेरेपी विकसित करने का मार्ग प्रशस्त हो सकता है, जो कि नुकसानदायक न हो।
स्कूल ऑफ बायोटेक्नोलॉजी, विज्ञान संस्थान, काशी हिन्दू विश्वविद्यालय के सहायक प्रोफेसर डॉ समरेंद्र कुमार सिंह के नेतृत्व में बीएचयू के शोधकर्ताओं की एक टीम ने इस माइक्रो आरएनए की खोज की है। अध्ययन के दौरान डॉसमरेंद्र सिंह और उनकी पीएचडी छात्रा गरिमा सिंह ने दिखाया कि एक मानव माइक्रोआरएनए, एमआईआर 34ए, वायरल ई6 जीन को खत्म करता है जो बदले में एक ऑन्कोजेनिक कोशिका चक्र कारक को बंद कर देता है और केवल सर्वाइकल कैंसर कोशिकाओं को मारता है।

उच्च जोखिम वाले मानव पेपिलोमा वायरस एचआर एचपीवी सर्वाइकल कैंसर के 99 प्रतिशत मामलों में कारक है जो होस्ट कोशिका के कई ट्यूमर प्रेसर्स और चेकपॉइंट कारकों को कमजोर करता है। वायरल प्रोटीन कई ऑन्कोजेनिक कारकों को भी स्थिर करता है, जिसमें एक आवश्यक कोशिका चक्र नियामक सीडीटी 2 डीटीएल भी शामिल है जो बदले में कोशिका परिवर्तन और प्रसार को बढ़ावा देता है।
डॉ समरेंद्र सिंह ने कहा, "माइक्रोआरएनए कोशिका चक्र और विभिन्न अन्य सेलुलर प्रक्रियाओं के एक महत्वपूर्ण नियामक के रूप में उभरे हैं। माइक्रोआरएनए में प्रतिकूल परिवर्तन को कई कैंसर और अन्य बीमारियों के विकास से जोड़ा गया है, लेकिन अभी भी उस तंत्र के बारे में बहुत कम जानकारी है जिसके द्वारा वे इन सेलुलर घटनाओं को नियंत्रित करते हैं।"
डॉ. सिंह ने कहा, हमने बताया है कि खोजा गया माइक्रोआरएनए कुछ प्रोटीनों को अस्थिर करता है और संक्रमित सर्वाइकल कैंसर सेल की वृद्धि को रोकने में भूमिका निभाता है, जिसके परिणामस्वरूप एचपीवी पॉजिटिव सर्वाइकल कैंसर कोशिकाओं के सेल प्रसार, आक्रमण और माइग्रेशन क्षमताओं को नियंत्रित करने में मदद मिलती है।

Cervical cancer is most common among Indian women after breast cancer, an expert said on Tuesday.
Talking to IANS, Dr Neerja Bhatla, Head Of Department, Obstetrics and Gynaecology, All India Institute of Medical Sciences, said that in Indian women, cervical cancer is the second most frequent cancer, but is preventable and curable, if found early and effectively treated.
However in India, an estimated 123,907 new cases and 77,000 deaths are reported each year, or one woman dying every 8 minutes, she said.
"Cervical cancer is unique in that it can be prevented by vaccination and has a long precancerous stage during which women can be tested and treated without suffering undue discomfort, anxiety, or financial hardship," she added.
The World Health Organisation has set goals to expedite progress toward the elimination of cancer and calls on countries to strive to reach the goals of vaccinating 90 per cent of girls with HPV vaccine by the age of 15, screening 70 per cent of women between the ages of 35 and 45 with a high-performance test such as the HPV test, and treating 90 per cent of women diagnosed with precancer or cancer, as well as facilitating palliative care, in the next decade.
However, the lack of awareness continues to be a key impediment to treatment participation and adherence. The Ministry of Health and Family Welfare has already started a screening programme for the three most frequent cancers: oral, breast, and cervical cancer.
On the occasion of International Women's Day, AIIMS Delhi screened 100 women using high precision HPV test. WHO recommends using a test that gives same day results.
This is the first time this strategy was tried at AIIMS using the GeneXpert platform that is also used for TB testing. It includes novel Point of Care HPV test and enabled Single Visit Screen and Treat approach. The report was available in only 60 minutes and based on the results, patients were offered treatment of precancer.

इस अध्ययन में रोगियों में बिना चीरफाड़ के सर्वाइकल कैंसर का पता लगाने का तरीका ढूंढ निकाला गया है। अध्ययन में डॉ. समरेन्द्र सिंह एवं उनकी टीम ने पाया कि सर्वाइकल कैंसर रोगियों के रक्त-नमूनों में सक्युर्लेटिंग सेल फ्री डीएनए की मात्रा बढ़ जाती है।
उन्होंने दिखाया कि कैंसर रोगी के रक्त नमूनों का उपयोग करके, नॉन-इनवेजि़व तरीके से (बिना चीरफाड़ के) वे रोगियों में ट्यूमर लोड का निदान कर सकते हैं और सर्जरी के परिणामों का भी अध्ययन कर सकते हैं।
सर्वाइकल कैंसर का पता लगा सकते हैं एवं इलाज की प्रगति को देख सकते हैं। यह अध्ययन रेडियोथेरेपी और विकिरण चिकित्सा विभाग, आईएमएस, बीएचयू के सहयोग से किया गया था।

What is Cervical Spondylosis?
Cervical spondylosis is a disorder in which there is abnormal wear on the cartilage and/bones of the neck (cervical vertebrae) and it is a common cause of chronic neck pain. It is a degenerative condition of the cervical spine affecting the middle and old age group of both sexes leads to pain and stiffness in the neck, radiating pain to the arm, paraesthesia, numbness, headache, giddiness, etc.
In Ayurveda, Cervical spondylosis is caused by a Vata dosha aggravation and it’s mainly a Vata problem. Its clinical presentation is similar to Vishwachi in which degenerative condition of the cervical spine is common. It’s a disease that mainly affects the neck and upper extremities having signs and symptoms similar to that of cervical spondylosis. Vishwachi is a vataja nanatmaja vikara. The involvement of Vata in the clinical manifestation of vishwachi is of prime importance.
Cervical Spondylosis Symptoms & Signs
Often develop slowly over time, but they get worse suddenly
Pain may be mild/it can be deep and so severe that affected movements
After standing/sitting at night, the pain gets worsen
Pain over the shoulder blade/it may spread to the upper arm, forearm, fingers
Pain gets worse at night/sneezing/coughing/laughing/bending the neck backward/walking more than a few yards
Muscles weakness
Neck stiffness that gets worsen
Abnormal sensation/numbness in the shoulders/arms/legs

Isometric cervical exercises may help to limit the loss of muscle tone. Molded cervical pillows can better align the spine during sleep & provide symptomatic relief for some patients. Ans. During the acute phase (about a week), usually complete bed rest & use of a thin pillow & hard bed is recommended. After acute phase, some panchakarma procedures like sadyovirechana, greevabasti, abhyanga, patra pinda sveda, nasya should be done.

New Delhi, Nov 16 (IANS) The World Health Organisation is set to launch a global strategy to eliminate the menace of cervical cancer, which is the second most common cancer in the Indian female population."On November 17, following the close of the 73rd World Health Assembly, WHO will mark this historic announcement and officially launch the elimination strategy. The moment has arrived for an ambitious, inclusive strategy to accelerate eliminating cervical cancer as a public health problem," it said.A WHO official told IANS that the strategy which would be unveiled on Tuesday was being prepared since 2018 under guidance of the organisation's Director General, Dr Tedros Adhanom Ghebreyesus, and has been finalised after many countries agreed to it.The WHO has recommended targets for 2030 to accelerate the progress towards elimination of cervical cancer where it suggested the countries to strive to reach the goals of vaccinating 90 per cent of girls of 15 years of age with HPV vaccine and screen 70 per cent of women at the age of 35 and 45 years.The All India Institute of Medical Sciences (AIIMS) informed that the Union Health Ministry had launched a screening programme for cervical cancer."The present strategy of visual inspection with acetic acid (VIA) has been rolled out in many districts. However, training of health workers, ensuring linkages between primary, secondary and tertiary level centres and access to services have posed challenges that are unique to every state. Public private partnerships are ongoing to further capacity building and implementation research," an AIIMS spokesperson said.The AIIMS also said that an indigenous HPV vaccine has been developed which is in Phase-3 trials now and is expected to be available in the near future. "The vaccine is developed by the Serum Institute of India in partnership with the Department of Biotechnology. It is expected to roll out by next year," an AIIMS official told IANS.The vaccine was launched back in 2006 and available globally. In India, it is available since 2009. "However, despite its availability, the vaccine is not taken up very well in India. Even the production is not adequate," said Prof Neerja Bhatla, of AIIMS' Department of Obstetrics and Gynaecology.She also hoped that the vaccine would be included in the national programme once its indigenous version is rolled out. "Let's hope it (vaccine) gets ready by the next year," Bhatla said.The current rate of disease burden is 14 patients per 100,000 population. "Our goal should be reduce it to less than 4," she added.Bhatla also informed that the cancer is mostly seen after 50 but the pre-cancer comes 10 to 15 years before. "So it will be prudent to get screened after 30 year of age so that the cancer is detected timely and treated," she added.Meanwhile, the AIIMS also said that it will hold a workshop with stakeholders on the launch of global strategy against cervical cancer. सर्वाइकल कैंसर को लेकर विश्व स्वास्थ्य संगठन (WHO) बेहद चिंतित है. उसके अनुसार सर्वाइकल कैंसर की रोकथाम और इलाज सफल होने की अधिक संभावनाओं के बावजूद हर साल 3 लाख से ज्यादा महिलाओं को कैंसर का यह रूप अपना शिकार बनाता है. हर एक मिनट में एक महिला में इस बीमारी की पहचान होती है.
विश्व स्वास्थ्य संगठन (WHO) ने कहा है कि सर्वाइकल कैंसर से मौत का शिकार होने वाली हर दस में से नौ महिलाएं गरीब देशों की होती हैं.
विश्व स्वास्थ्य संगठन (डब्ल्यूएचओ) के अनुसार अगर इसे रोकने की दिशा में कोई कदम नहीं उठाया गया तो 2040 तक इस बीमारी से होने वाली मौतों में लगभग 50 प्रतिशत की वृद्धि हो जाएगी.
बीमारी की रोकथाम के लिए यह सुनिश्चित करना ज़रूरी है कि विश्व स्तर पर 9-14 वर्ष की सभी लड़कियों को ह्यूमन पेपिलोमावायरस (एचपीवी) वायरस के ख़िलाफ़ टीके लगाए जाएं.
एचपीवी ऐसे वायरस का समूह है जो कि बेहद आम है और जिसके दो प्रकार सर्वाइकल कैंसर के 70 फ़ीसदी मामलों के लिए ज़िम्मेदार हैं.
विकासशील देशों की महिलाओं की रोकथामकारी उपायों तक सीमित पहुंच होती है और सर्वाइकल कैंसर का अक्सर तब तक पता नहीं चलता, जब तक वह विकसित चरण तक नहीं पहुंच जाता.
अंतिम चरण में सर्वाइकल कैंसर का उपचार- जैसे सर्जरी, रेडियोथैरेपी और कीमोथैरेपी - सुविधाओं की उपलब्धता कम है जिसके कारण इन देशों में बड़ी संख्या में महिलाओं की मृत्यु हो जाती है.
डब्ल्यूएचओ का कहना है कि स्थिति को बेहतर करने के लिए नई तकनीक और रणनीतियों, निदान और तेज़ी से फैलने वाले कैंसर के शुरुआती चरण में इलाज की भी जरूरत है. इसके अतिरिक्त इस बीमारी से ग्रस्त होने वाली महिलाओं के लिए तकलीफ दूर करने के उपाय सुनिश्चित किए जाने चाहिए.
सार्वभौमिक स्वास्थ्य कवरेज देने वाली सशक्त स्वास्थ्य प्रणालियों में इन सेवाओं को शामिल किया जाना चाहिए. सभी को समान रूप से ऐसी सेवाएं उपलब्ध हों, इसके लिए राजनैतिक प्रतिबद्धता, अधिक अंतरराष्ट्रीय समन्वय और सहयोग की जरूरत होगी. अनेक देश और यूएन एजेंसियां संयुक्त राष्ट्र साझा रूप से विश्वस्तरीय सर्वाइकल कैंसर रोकथाम और नियंत्रण कार्यक्रम का हिस्सा बन गए हैं. इस पांच वर्षीय कार्यक्रम के तहत विभिन्न देशों की सरकारों और उनके सहभागियों को राष्ट्रीय सर्वाइकल कैंसर नियंत्रण कार्यक्रम तैयार करने हेतु विश्वस्तरीय नेतृत्व और तकनीकी सहयोग प्रदान किया जाता है. सार्वजनिक स्वास्थ्य के लिए बड़ी चुनौती बन कर उभर रहे सर्वाइकल कैंसर को समाप्त करना इन कार्यक्रमों का उद्देश्य है. डब्ल्यूएचओ के अनुसार इस बीमारी पर नियंत्रण पाने के लिए सरकारों, संयुक्त राष्ट्र एजेंसियों, अनुसंधानकर्ताओं, स्वास्थ्य सेवा कर्मचारियों और व्यक्तियों के साथ ही जीवन रक्षक दवाओं, और निदान और उपचार यंत्रों के निर्माताओं की भी महत्वपूर्ण भूमिका निभानी होगी.
