Fistula
Overview
Fistula is considered to be a hollow tract or cavity lined by one opening (external) in the skin and the internal opening in the anal region.
In ayurveda it is known as ‘Bhagandar’ which means a serious illness where pain is caused to the ‘bhag’ area which means the perineal region.
An abscess which is not treated at the initial stage converts into a tunnel like structure along with fluid discharge from the tract. According to acharya sushruta improper food habits and daily regimen leads to disturbance of vata dosha(Air)in the anal canal which leads to disturbance of mamsa dhatu(muscular system) and rakta dhatu(blood tissue) and gives rise to a boil or abscess. If this boil or abscess remains untreated it leads to oozing of discharge of fluid, this is called as ‘Bhagandar’. Few other reasons are also responsible for causing bhadangar and those are krimi (Microorganisms) caused due to a trauma, straining, sitting for the toilet for a longer duration of time.
Types of Bhagandar
- Satonaka: a fistula having more than one opening.
- Ustragreeva: curved fistula, shape resembling the neck of camel
- Parisravi: fistula with big opening and profuse discharge.
- Shambhukavart: fistula resembling the horseshoe.
- Unmargi: fistula caused due to trauma.

Signs and symptoms of anal fistula:
- Frequently anal abscesses
- Pain and swelling around anal region
- Bloody and foul full discharge around the anal region.
- pruritis ani Itching around anal drainage
- Tenderness around the anus
- Thick discharge keeps the blister area remaining wet.
- Pain during bowel movement
- Bleeding is continued from the opening
- Fever chills and general feeling of fatigue.
Causes of anal fistula:
Anal fstula connects infected anal glands to soft lining of anal region, these glands may become clogged and infected. An infected gland is called as an abscess and an abscess creates a pathway in external surface to drain the pus. Once drained it may reoccur again if proper cleaning and hygiene is not maintained.
Other conditions that affect inflammation in anal region or lower digestive area can increase risk for fistulas. Some of the condition includes:
- Crohn’s disease results from inflammation of the intestine.
- Cancers and its treatments
- Trauma due to foreign body insertion
- sexually transmitted disease such as chlamydia and syphilis.
- Tuberculosis of intestines.
Management of fistula by home remedies:
- Sitz bath: a sitz bath includes soaking and fomenting the anal region in warm water to bring reliuf from irritation, swelling pain. The patient must sit for 20-30 mins in tub filled with warm water mixed with betadine.
- Stool softener: Take 1 tablespoon of cow ghee in a glass of cow milk lukewarm in order to enhance digestion and soften the stools. It can be used as homemade natural laxative or stool softener.
- Ointment: use coconut oil, aloe vera or calamine solution as natural ointment to reduce the pain, itching and burning sensation from the anal region.
- Hydration: maintain proper hydration level in the body for which you drink plenty of water in order Av to keep the stools loose and avoid constipation.
- Avoid Oily and spicy foods: Oily and spicy foods worsens the inflammation by irritating the intestine and causing bleeding so it must be either stopped or reduced.
- Increase fibre intake in diet: fibrous foods must be included in diet for better digestion and producing soft bowel removal from the anus preventing the irritation of the fistula.
- Personal hygiene: Personal hygiene has a major role in fistula management, take a good care of intimate hygiene. Wash the anal region gentally use a gentle toilet paper or cotton cloth to clean the region. Clean hands before cleaning the anal region.
